If your student group is required to provide certification of student members' academic or registration status at the University of Minnesota in order to participate in off-campus activities, follow these steps.
- Gather eligibility documents. These are any documents required to be completed and submitted to the sponsoring organization.
- Complete the Consent to Release Academic Information form. Each group member will need to sign it in ink. This permits the University to release academic information needed to verify eligibility for academic and or scholarship awards programs.
- Once you've received all signatures and completed all form fields, send the form and eligibility document(s) to [email protected] or drop it off in person at a One Stop office.
When we've processed your form, we'll send a confirmation email to the contact person listed on the form.
Once the Office of the Registrar receives the form, it will take 1-3 business days to process the request. It may take longer if you submit incomplete information. Please contact the Office of the Registrar at [email protected] with any questions.
