This form enters you into a contractual agreement with the University of Minnesota as a participant in the Nurse Faculty Loan Program (NFLP).
Note: Section 3 must be completed by an authorized official from the borrower’s place of employment.
Use this form if...
- You will be or have been employed full-time as nurse faculty in the School of Nursing for a complete year.
Don't use this form if...
- You have not been or will not be employed for a year in the School of Nursing or you do not have a Nurse Faculty Loan.
