This is a form for veterans, service members, and dependents to request Department of Veterans Affairs benefit certification for courses taken at the University of Minnesota.
To complete this form, you will be asked to provide:
- Completed application for benefits with the VA
- Certificate of Eligibility (COE) (if already received)
Privacy notice: This information is being requested in order to determine your eligibility for Veterans Benefits at the University of Minnesota and will be viewable by University staff with a business need. You are legally required to submit the information and/or documentation in order to receive Veterans Benefits. Refusal to submit the information will result in your courses at the University not being certified for your Veterans Benefits.

- You applied for benefits with the U.S. Department of Veterans Affairs (VA)
- You received or will receive your Certificate of Eligibility (COE).
- You applied for Chapter 31 VR&E benefits
Don't use this form if...
- You have not applied for benefits with the VA
- You have not been approved for Ch 31 VR&E benefits
- You intend to only use non-VA benefits, like Federal Tuition Assistance, Minnesota GI Bill, and State Tuition Reimbursement
