Use this form if...

  • You have been notified by the UMN Office of Student Finance via email and financial aid checklist in MyU that your financial aid application was selected for the verification process and you are an independent student.

Don't use this form if...

  • You are a dependent student.
  • You have not been notified by the Office of Student Finance to complete verification. 

Next steps

You may receive a separate email to your university email requesting additional tax documentation or employment confirmation. Please follow all instructions included in those communications.

Please note your verification form will remain in initiated status in MyU until reviewed by the Office of Student Finance. Do not resubmit this form.
