This form is used if you're unable to verify your identity in person with a One Stop counselor in order for your financial aid application to be processed.
To verify your identity, you must present a valid U.S. government-issued photo identification (e.g., a driver’s license, other state-issued ID, or passport) and sign the statement in SECTION B in the presence of a licensed Notary Public. You must drop off the completed form along with a copy of your government-issued photo ID in person or mail it to One Stop Student Services.
Use this form if...
- The UMN Office of Student Finance notified you that you need to verify your identity for your financial aid application.
- You're unable to verify your identity in person with a One Stop Counselor.
Don't use this form if...
- You are not applying for financial aid.
- The UMN Office of Student Finance has not requested that you complete this form.
