This form is used to request resident tuition rates for employees and their immediate family members on the basis of appointment classification and percentage of appointment. This form must be completed for each term an employee or their family member wishes to request this benefit.

Use this form if...
You/your family member are classified as nonresident for tuition purposes and you meet one of the below criteria:
- Hold an appointment in 93xx, 94xx, 96xx, 97xx, 9540, 9546, 9548, 9552, 9555, 9556, 9560, or 9569 and have a 75 percent appointment for the entire term of eligibility.
- Academic employees with an appointment in classes 9545 or 9558 and hold a minimum of 25 percent appointment for the entire term of eligibility.
- Civil service/Represented bargaining unit employees holding appointments in classes lxxx-8999 and hold a minimum of a 75 percent appointment for the entire term of eligibility.
Don't use this form if...
- You are a Graduate Assistant
- You do not meet criteria listed on the form/above.
