This is a request for undergraduate students who do not want to enroll for one or more terms but wish to return within two academic years. 

Before completing this form, review One Stop’s leave of absence information and discuss with your college advisor whether requesting a leave of absence is appropriate.

  • You plan to remain enrolled
  • You are not an undergraduate student
