This form is an application for undergraduate students to transfer from one University of Minnesota Twin Cities or Rochester college to another UMN Twin Cities or Rochester college.
This form is also for students who want to earn more than one degree (e.g., BA and BS) and the second degree is in a different college.
You can only apply to one college per application. If you need to submit applications for multiple colleges, please fill out a separate application for each college.
Before you complete this form, discuss your interest in changing colleges or adding an additional degree with your academic advisor.

Use this form if...
- You are a current or previously enrolled University of Minnesota Twin Cities or Rochester student, and you want to change your current college to another UMN Twin Cities or Rochester college.
- You want to apply for an additional degree in a college you are not currently enrolled in. For example, you are earning a BA in Anthropology (CLA) and wish to add a BS in Kinesiology (CEHD).
Don't use this form if...
- You are not an undergraduate student or are not currently or previously enrolled at the University of Minnesota Twin Cities or Rochester.
- You are interested in changing your major and plan to remain in your current college. This should be discussed with your academic advisor.
