This is a tuition waiver for legally blind undergraduate, graduate, and non-degree seeking students who are Minnesota residents.
To complete this form, your physician, ophthalmologist, and/or campus Disability Resource Center (DRC) Access Consultant must complete and sign SECTION B.
Privacy notice: This information is being requested in order to determine your federal, state, and/or institutional financial aid eligibility at the University of Minnesota and will be viewable by University staff with a business need. Declining to submit the information will prevent the University from finalizing your aid eligibility. You are not legally required to submit the information and/or documentation. However, refusal to do so will result in an incomplete financial aid application, and therefore, you may not receive financial aid for which you applied.
Use this form if...
You meet all of the following requirements:
- Legally blind (vision is no better than 20/200 or 20 degrees of visual field in the better eye)
- Minnesota resident who meets the University’s definition of residency
- Enrolled in a credit-bearing course(s) that charge(s) tuition NOT enrolled in a fee-based program, study abroad program or National Student Exchange with host pay option.
