This form is an appeal to refund tuition and associated fees for courses you withdrew from for rare or extreme circumstances, such as:
- Medical issues
- Death in the Immediate Family
- Military Activation
- Incorrect Academic Advisement
A tuition refund appeal does not appeal housing, dining, parking, or other charges that are not related to your tuition.
Please talk with your advisor and One Stop about whether a tuition refund is an option for you.
In order to submit this form you must officially withdraw from the courses you would like to appeal, and provide a personal statement explaining the circumstances.
Documentation must be provided as evidence of your rare or extreme circumstances for withdrawing. Documentation depends on your situation. Types of documentation may include:
- Medical Supplement.
- Proof of military activation.
- Death certificate.
- Official letter from your college or advisor.

Use this form if...
- You would like to request a tuition refund for a term within the last calendar year. (For example, if you are appealing Spring 2026, you have until the end of Spring 2027 to appeal.)
- You have withdrawn from the course(s) for which you would like to appeal.
Don't use this form if...
- You are still enrolled in the courses you would like to appeal.
- The term you want to appeal was more than one calendar year ago.
- You are appealing to have a “W” removed from your academic record.
- You are appealing tuition for courses charged at the University of Minnesota Duluth, Morris, or Crookston.
- You want to appeal student health insurance, dining, bookstore, Course Works, or housing services. Contact those services directly.
If you have already had a tuition refund appeal approved for the same or similar circumstances, another appeal for the same reason is unlikely to be approved.
