Tuition Assistance for Deaf/Hard of Hearing Students

This form is to request tuition assistance for students who are deaf or hard of hearing.

To complete this form, your physician, audiologist, and/or campus Disability Resource Center (DRC) Access Consultant must complete and sign SECTION B.

Use this form if...

You meet all of the following requirements:

  • Minnesota resident who meets the University’s definition of residency
  • Enrolled in an undergraduate degree-seeking program
  • Not enrolled in a study abroad program
  • Have a hearing loss of such severity that you are primarily dependent on visual communication, such as writing, lip reading, manual communication, and gestures.
  • Will receive a Pell grant and/or Minnesota State Grant based on the credits in which you are enrolled during the term in which tuition assistance is applied.

Don't use this form if...

  • You are not a Minnesota resident
  • You are not enrolled in an undergraduate degree-seeking program
  • You are not primarily dependent on visual communication due to the severity of your hearing loss
  • You are not receiving a Pell grant and/or Minnesota State Grant

Or:

  • You are enrolled in a study abroad program.
