This form is used to request assessment of the Student Services Fee. By completing this form, you are requesting to be charged the Student Services Fee, which provides students access to services like Boynton Health Services, the RecWell Center, Student Legal Services, and more.

This form should not be submitted until your registration is complete.
Use this form if...
- You have not been charged the Student Services Fee and wish to be charged the fee.
Don't use this form if...
- You are part of one of the ineligible programs for this fee:
- Regents Tuition Benefit Program
- Senior Citizen Education Program
- Post-secondary enrollment options (PSEO) or concurrent high school enrollment program
- You plan to adjust your enrollment to be registered in 6 (3 in summer terms) or more credits for the current term, as you will then be automatically assessed this fee.
- You only wish to pay part of the Student Services Fee for a service. It is not possible to select part of this fee. You either pay the whole cost for access to one of these services or do not pay the fee and do not have access to the services the fee provides.
- You are requesting the Student Services Fee to be waived. As a result of the Student Services Fee policy changes approved in June 2024, there is no longer a Student Services Fee waiver. All students enrolled in 6 or more credits (3 or more credits in the summer) must pay the Student Services Fee, regardless of course modality. Programs may request an annual exemption, check with your program office for more information.
