This form authorizes the University of Minnesota to release student record information to third parties. A separate form must be filled out for each third party to whom you grant access to information on your student record.
Use this form if...
- You want to give the University permission to release information about your student records to a third party, such as parents, a spouse, or a sponsor.
Don't use this form if...
- You can grant parent/guest access in your MyU.
- You are looking to grant a third party power of attorney.
