Student Athlete Outside Aid Report

This is a form for the University to collect information about outside awards given to student athletes. This form must be completed separately for each scholarship you receive.

Use this form if...

  • You are a student athlete, and
  • You have received financial aid awards from an agency other than the University of Minnesota Twin Cities. 

Don't use this form if...

  • You are not a student athlete, and
  • You have not received financial aid awards from agencies outside of the University of Minnesota Twin Cities.

Next steps

After you and the awarding agency have completed this form, it will be routed to the Athletic Compliance staff for review and processing. Processing may take 3-5 business days.
