Return to the University for Undergraduate Programs

This form is an application to request to return to the University of Minnesota - Twin Cities undergraduate programs after a leave of absence, being discontinued, or academic suspension. 

Information required to complete this form includes residency information, work/volunteer/military experience, and additional education information/transcripts. If you have new coursework since you last attended UMN, submit the official transcripts to the Office of Admissions.

International students: Contact International Student and Scholar Services (ISSS) before you fill out this form. There may be additional steps you need to take. You can reach ISSS at [email protected] or 612-626-7100.

You must submit this form to return to a University college no later than 3 weeks before the start of the term for which you plan to enroll. If you plan to return to a University school (like the School of Nursing or Dentistry), special circumstances may require an exception to these deadlines. Please contact the school office for specific application deadlines.

Use this form if...

  • While previously attending the University of Minnesota you were a degree-seeking undergraduate student.
  • You are currently discontinued (inactive status), on a leave of absence, or academic suspension.
  • You wish to return to your last enrolled undergraduate program.

 

Don't use this form if...

  • You have  not previously been an undergraduate student at the University of Minnesota.
  • You have not previously been enrolled at the University of Minnesota - Twin Cities.
  • You do not wish to return to your last enrolled undergraduate degree program.
  • You want to apply to a new program at the University of Minnesota - Twin Cities.

Completed forms are routed to the appropriate college, school, or center to review for readmission decisions which may take up to three weeks. For questions about returning to the University of Minnesota, contact your college or department office.

If you are returning from academic suspension, you must contact your college’s student services office to determine what other actions are required.
