Use this form if...
- One Stop Student Services sent you notification via UMN email to complete and return the W-9S form because your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) is not on file with the University of Minnesota.
- You are updating from an ITIN to a new SSN.
Don't use this form if...
- You already have a current Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) on file.
