Use this form if...
- You earned a bachelor’s degree elsewhere after your University of Minnesota suspension, and
- You return to the University as a non-degree seeking student in CCAPS
Don't use this form if...
- You have not earned a degree elsewhere after being suspended from the University of Minnesota
- You wish to return to complete a degree at the University of Minnesota. In this case, you must work directly with the college that suspended you to discuss returning from suspension.
