Include a permission number if necessary (see drop/add deadlines to see if you need instructor approval to register— a permission number is not required for dropping a class).

- You are experiencing issues registering in your MyU.
Don't use this form if...
- You are not admitted into a program or have not activated your record as an undergraduate non-degree-seeking student.
