Once during a student’s undergraduate career, they may withdraw from a course and receive a “W” on their transcript, after the deadline for withdrawal, without documented extenuating circumstances. This form may be used up to and including the last day of instruction for the course. A student may not submit this form after the final or its equivalent has been completed.
Use this form if...
- You are an undergraduate student
- You would like to withdraw from a course (and receive a “W” on your transcript) after the course’s withdrawal deadline
- You have not utilized your one-time only late withdrawal in your undergraduate career
- You have not completed the final or equivalent for the course
