Graduate students who experience circumstances that prevent them from maintaining active status through continuous registration (excluding summer term) and who, through consultation with their Director of Graduate Studies (DGS), advisor(s), and relevant offices determine that a leave of absence (LOA) is appropriate, must request a LOA from their college using this form.
This form requires signatures from your advisor(s) and DGS(s).
You are expected to request a LOA prior to the beginning of the proposed term/year of the leave.

Use this form if...
You are:
- a graduate student,
- experiencing circumstances that prevent you from maintaining active status through continuous registration,
- have reviewed Administrative Policy,
- and have consulted with your DGS
