Language Department Certification of Foreign Language Proficiency

This form certifies that a student has demonstrated language proficiency for an official transcript by:

  • General extension special course examination
  • Course credit from another institution
  • Demonstrated higher proficiency
  • University of Minnesota course credit
  • Graduate reading exam from another institution
  • Or some other means approved by the language department

Use this form if...

  • You have demonstrated proficiency in a language offered by the University of Minnesota and wish to have the proficiency marked on your official transcript.

Don't use this form if...

  • You have not successfully demonstrated language proficiency.
  • You do not want to have your proficiency marked on your official transcript. 

Next steps

Once this form is completed, submit it to your graduate program office. Note that this form requires the signature approval of an authorized language department official. 

Your program office will route this form to Graduate Student Services & Progress (GSSP) for processing.
