Incomplete Grade Contract

An incomplete “I” grade can be assigned at the discretion of an instructor. Incompletes may be assigned when extraordinary circumstances prevent a student from completing coursework on time. 

This form is a contract between a student and instructor that outlines the requirements to complete the class. 

This contract should be initiated by you after a conversation with your course instructor about completing a class with an Incomplete Grade. You should either have an “I” grade (incomplete) or no grade assigned for the class in order to complete this form.

Use this form if...

  • You and your instructor have agreed to an incomplete grade
  • You are an undergraduate student
  • You are a graduate or professional student and you have been instructed to complete this.

Don't use this form if...

  • You and your instructor have not agreed to an incomplete grade
  • You are a graduate or professional student and your department or college has their own incomplete contract process.
