Graduate Student Petition

This form should be used by students who need to make changes or updates to a previously submitted Graduate Degree Plan (GDP).

You will need to obtain the signatures of your advisor(s), Director of Graduate Studies, and college. The form may require additional signatures if you have a minor.

Use this form if...

You want to request to do the following:

  • Change coursework on your approved GDP, including transfer work
  • Change or remove the language requirement on your approved GDP
  • Add or remove a minor on your approved GDP

Don't use this form if...

  • Your program uses the Graduate Planning & Audit System (GPAS).
