Graduate Registration Exception Request

This form is used to obtain an exception or other accommodation to college or University academic policies for graduate students. 

Your request will be reviewed by your graduate program and other University offices, as necessary.

You are not legally required to submit this information, however declining to submit the information will prevent the University from accurately reviewing your request and taking action.

  • You are an active graduate student requesting an exception to the University academic policies.

Don't use this form if...

  • You are not an active graduate student
  • You can successfully complete the requested action through MyU.

Next steps

If you are petitioning for medical reasons, have your medical provider or the Disability Resource Center complete a Medical Supplement to document these circumstances. You will be prompted to upload support documentation on this form.
