Graduate Degree Plan

This form is used to outline and declare the courses you plan to use toward your graduate degree. The Graduate Degree Plan is phased out and most students should complete a Graduate Planning & Audit System (GPAS) Planner.

This form requires your advisor(s), Director of Graduate Studies, and the College’s signature. You may need additional signatures as outlined on the form if you have a graduate minor.
