Use this form if...

  • You are an active or recently graduated graduate student, and
  • Your degree requirements have been or will be completed by the degree date you are requesting. 

Don't use this form if...

  • You need to change your expected graduation term or month after you've applied to graduate, but before you've graduated. Instead, please email [email protected]. 

Next steps

Your request will be reviewed by a committee within one week. The review committee will let you know a final decision by email.
