Student group academic eligibility certifications

If your student group is required to provide certification of student members' academic or registration status at the University of Minnesota in order to participate in off-campus activities, follow the instructions below:

1. Gather eligibility documents

These are any documents required to be completed and submitted to the sponsoring organization.

2. Complete the Consent to Release Academic Information form

Your group will need to complete this form, and each member will need to sign it. This permits the Office of the Registrar to release academic information needed to verify eligibility for academic and or scholarship awards programs.

3. Submit the Consent to Release Academic Information for and eligibility document(s) via email or in person at

4. Email sent to contact listed on the release form

You will receive an email with your completed forms. Normal processing is 1-2 business days. Delays may occur if you submit incomplete documentation.
