This form is used to transfer University of Minnesota coursework from undergraduate to graduate career levels.
The credits must be approved by the undergraduate and graduate advisors to confirm they are appropriate for transfer to the graduate career level. Additionally, instructor signatures are needed for each course unless are in a Regents-approved, integrated bachelor/master’s program.
Individual classes are not moved from the undergraduate transcript to the graduate transcript. The credits appear under “Transfer Credits” at the top of the graduate transcript.

Use this form if...
You want to use coursework you took as an undergraduate student toward a graduate degree.
Please note, to transfer credits to a graduate career level:
- Credits may not be used twice (i.e. no double counting)
- Undergraduate degree must be awarded before starting another degree
- Classes must be completed and graded
- Student must already be admitted and active in the target career
